Can someone help me ID this? I've had this cactus for almost a year, and it's already this tall.

Can anyone inform me on whats going on here? I have had him for al lest 4 to six months and its never been like this. i just moved but i dont think thats why. i also checked its stem and roots for rot but there is non. just the leaves turning black brown and drying up and dying..please help! its my favorit one.
